Introduction : A ce jour, les indications précises d’ostéotomie fémorale distale de varisation (OFV) restent encore à préciser, en particulier vis à vis d’une arthrose fémoro-patellaire (AFP) associée. L’objectif de cette étude prospective est d’évaluer le retentissement fonctionnel et radiologique de l’OFV par soustraction interne sur l’articulation fémoro-patellaire chez des patients souffrant d’une arthrose fémoro-tibiale externe (AFTE) associée à une arthrose fémoro-patellaire (AFP) sur déformation axiale en genu valgum.
Matériel et méthode. 14 patients (15 genoux) ont bénéficié d’une OFV par soustraction interne. Le retentissement fonctionnel de l’OFV sur l’articulation fémoro-patellaire a été évalué par le score KOOS-PF et le score de Kujala. Le recentrage de la patella a été mesuré par l’angle de congruence fémoro-patellaire de Merchant. Les scores fonctionnels pré et post-opératoires ont été recueillis prospectivement et comparés à un an de recul post-opératoire.
Résultats. Les scores fonctionnels fémoro-patellaires de Kujala et le KOOS PF étaient significativement améliorés avec un différentiel respectivement de +37,5 points ± 20,4 et +42,7 points ± 19,3 (p<0,01). L’angle de congruence de Merchant moyen est passé de 8,7° en externe à 0,3° en interne, avec pour conséquence une médialisation de la patella, sans toutefois retrouver de différence significative (p = 0,2).
Discussion. Bien que le seuil de significativité concernant l’angle de congruence de Merchant et donc la médialisation de la rotule n’ait pas été atteint, l’OFV par soustraction interne améliore à court terme la symptomatologie fémoro-patellaire chez des patients souffrant d’une AFTE associée à une AFP sur déformation axiale en genu valgum.
Introduction. To date, specific indications for distal femoral varus osteotomy (FVO) have yet to be clarified, especially in cases of associated patellofemoral osteoarthritis (PFO). The purpose of this prospective study is to assess the functional and radiological impact of a medial closing-wedge femoral varus osteotomy on the patellofemoral joint in patients with valgus deformities who are afflicted with lateral tibiofemoral osteoarthritis (LTFO) associated with PFO.
Materials and method. 14 patients (15 knees) received a medial closing-wedge femoral varus osteotomy. The functional impact of an FVO on the patellofemoral joint was assessed based on the KOOS-PF (Knee Injury and Osteoarthritis Outcome Score – Patellofemoral Subscale) and the Kujala score. Realignment of the patella was measured by the Merchant’s patellofemoral congruence angle. The pre- and post-operative functional scores were compiled prospectively and compared one year after the surgery.
Results. The Kujala patellofemoral functional scores and the KOOS-PF showed considerable improvement with a differential of +37.5 points ± 20.4 and + 42.7 points ± 19.3 (p<0.01) respectively. The average Merchant’s congruence angle went from 8.7° laterally to 0.3° medially, resulting in medialization of the patella, yet failed to reveal any significant difference (p = 0.2).
Discussion. Although the threshold of significance for Merchant’s congruence angle, and therefore the medialization of the patella was not reached, a medial closing-wedge femoral varus osteotomy improves short-term patellofemoral symptoms in patients with valgus deformities who are afflicted with LTFO associated with PFO.
